After the on-site survey, the surveyors complete the survey report forms and
other survey documents and submit the documents to IMQ for preparation.
Accreditation decisions are made by the Corrections and Detentions Health Care
Committee (CDHCC), comprised of seven physicians, one registered nurse, and one
mental health professional, who meet every two months.
Accreditation determination is based upon standards being met. A facility or
program not meeting an adequate percentage of standards may be given the
opportunity to submit additional documentation for the Committee to review at
its next meeting. A facility is notified in writing of the accreditation
decision, along with a request for any additional documentation or a framed
certificate of accreditation if accreditation has been awarded.
The accreditation decisions that may be rendered are:
- Two-Year Award – Meeting 100% Essential and 85% Important Standards
- 90-Day Accreditation Probation
- Denial – Requiring Re-Survey
